To support priorities in our area, the HCP has been delivering positive change in several areas.
One of these areas is the virtual hospital. The hospital was started at the beginning of the COVID-19 pandemic to care for people in their own home. It was awarded the BMJ Respiratory Team of the Year in recognition of the innovation.
The hospital has since evolved into a highly successful integrated service, run by the providers in the South and West Hertfordshire HCP. The care it provides now extends to people with chronic obstructive pulmonary disease (COPD) and heart failure.
Other priority areas include:
- diabetes
- frailty
- advanced care planning
- children and young people’s services.
The HCP is also passionate about making sure those who live in south and west Hertfordshire have their say in what the partnership does and what changes need to be made. We are working with Healthwatch Hertfordshire to put in place some productive ways to involve local people and design changes with them.
Over the longer term, our partnership wants to reduce health inequality, prevent people from becoming ill, and ultimately improve the health and wellbeing of people who live in this area.
Involving local people and communities
The South and West Herts Health and Care Partnership Co-production Board is made up of local patient volunteers and representatives from health, social care and voluntary sector organisations, works to improve people’s health and wellbeing and ensure communities receive the care they need.
There are patient volunteers already involved with this work. Further requests for volunteers will be advertised here.
