Priorities
We will provide early help to our residents to prevent mental illness and support the health and wellbeing of those with a Severe Mental Illness (SMI), learning disabilities or autism. We will seek to achieve the following outcomes:
Outcomes we want to achieve for our residents through delivering this priority expressed as “I” statements
As a resident:
- I am supported to have good mental health.
- I know how to access activities that promote my wellbeing.
- My care and support are based on my needs and wishes, and not others.
- I see the right professionals for my health and care.
- I can access support through digital and online options should I want them.
- I am supported to think about what will happen in the short, medium, and long-term.
- I have access to employment and volunteering opportunities.
- My school/education setting, or workplace understands my neurodiversity and makes reasonable adjustments.
- My family and I are supported to get a diagnosis of my condition and support is provided whilst I await diagnosis.
Potential key performance measures
- Indicator 1 – % difference in average age of death between people with a learning disability and SMI compared to general population.
- Indicator 2 – % of people with mental-ill health and/or autism or LD in education or employment.
- Indicator 3 – % emergency admissions for patients with a mental health diagnosis.
- Indicator 4 – Proportion of people with mental ill health known to the criminal justice system.
- Indicator 5 – % rate of suicide amongst young people with SMI (Metric Suicide rate (persons aged 10+)
- Indicator 6 – % of people with LD with an annual health checks and Heath Action Plan.
- Indicator 7 – Proportion of waiting times for both Children and Young People and adults to receive autism and ADHD diagnosis.
